REQUIREMENTS FOR ADOPTION
1) Letter from Barrister/Solicitor to the Chief Social Services Officer or The Director of Children’s Affairs
2) Case History of Child/Children to be adopted –2 copies
3) Child/Children four (4) Passport Pictures
4) Two copies Child/Children Certificate
5) Two copies of Death Certificate of any of the biological Mother of Father if deceased
6) Consent of Parents of Guardians (consent to be signed in the presence of CSSO/Director of Children’s Affairs)
7) Parents/Guardian Passport, National Voter I.D. Card
8) Pre‐Adoption counseling with biological Parents or Guardian
